If your child requires medication during the school day, please complete the Student Medication Form.

All students enrolling at CLMS must have on file:

• A copy of child’s Birth Certificate submitted within 30 days of enrolling the student
• Child History Questionnaire
• Emergency Card
• Photo Release
• Student Health & Safety Permission
• Students are required to have had a health exam performed within the following time frames:
• within one year prior to entering K or 1st grade;
• upon entering the 6th and 9th grades, or
• irrespective of grade, immediately prior to or upon entrance into any school.
• Eye exam for students entering Kindergarten, or any grade if this is the first time the student has enrolled in a school in Illinois turned in by October 15th
• Dental exam for students in Kindergarten, 2nd, and 6th grade students turned in by May 15th

Students cannot attend school after October 15th if requirements for health examinations have not been met. If you have a religious exemption or are unable to schedule an exam, there are waiver forms.

Annual hearing and vision screening for preschool, 6th, and 8th grades is performed annually as required under Illinois State law. This is not a substitute for the Eye exam requirement.
